The postnatal management of severe congenital diaphragmatic hernia (CDH) remains challenging with few robust evidence-based interventions but many therapies based on case reports or small case series. A good example of this is sildenafil, a phosphodiesterase inhibitor, which is increasingly used in the management of pulmonary hypertension associated with CDH.1 2
